Significant outcome changes included a reducti= on in time to extubation from the start of the operation (24.1 hours versus 1= 1.5 hours), a shorter ICU length of stay (3.9 days versus 2.6 days), and decre= ased hospital length of stay (10.3 days versus 6.4 days) between 1991 and 2001, respectively.  While no diffe= rence was found in preoperative beta-blockade, significant differences were note= d in intraoperative and postoperative beta-blocker usage (p < 0.05).  Improvements in time to nasogastr= ic tube removal and time to restarting of both clear and solid enteral feeding also achieved statistical significance (p < 0.05).   No difference in post-opera= tive renal function was seen between the two groups.

DISCUSSION:
The results of this pilot study demonstrate a statistically significant decrease in intraoperative crystalloid administration and increase in perioperative beta-blocker usage between 19= 91 and 2001.  This appears to correlate to shortened times to extubation and return of enteral function,= as well as decreased lengths of both ICU and overall hospital stay between patients who underwent AAA repair between 1991 and 2001.  While limitations of this retrosp= ective study prohibit determination of a cause and effect relationship, the data = are representative of active anesthetic management changes which have evolved = over the past decade.  Changes in healthcare administration could be a confounding variable in interpreting diminished length of hospital stay but are unlikely to account for improved clinical measures such as time to extubation and return of enteral function.  While rates of postoperative respiratory failure, pneumonia, and mortality did not reach statistical significance between the two study periods, future analysis of= a larger sample size might allow for improved interpretation of the data.
<= b>INTRODUCTION:
Common problems following surgical repair of abdominal aortic aneurysms (AAA) include respiratory failure, prolonged intubation, pneumonia, atelectasis, and delay to enteral feeding.  Despite the known correlation bet= ween excessive volume administration, and the increased risks of pulmonary complications and abdominal visceral edema, many institutions still admini= ster large volumes of fluids that often exceed 1,000 milliliters per hour. Evol= ving care at our institution in Cleveland, Ohio has resulted in limitations on intraoperative crystalloid administration and an increase in perioperative beta-blocker use. To quantify these management changes and the resulting patient outcomes, we performed a retrospective chart review of elective infrarenal AAA repair patients
METHODS:
After obtaining University Hospital= s of Cleveland IRB approval, we generated patient lists using billing codes for elective infrarenal AAA repair from 1991 and 2001. Variables examined incl= uded patient age, gender, intraoperative fluid administration (i.e.: crystalloi= d, blood products, or colloids), and beta-blocker administration. Outcome measures evaluated included perioperative renal function, time to extubati= on, length of ICU stay, hospital length of stay, and parameters of post-operat= ive return of enteral function (time to NG tube removal, time until clear liqu= ids begun, and time until solid food restarted).  Statistical analysis was done usi= ng SPSS software to perform an ANOVA.
RESULTS:
A total of 43 patient charts were analyzed, including fifteen patients from 1991 and 28 patients from 2001.  The average age at time of AAA re= pair was 66.2 years old in 1991 and 70.9 years old in 2001.  Perioperative fluid administration averaged 6,982 milliliters per patient in 1991 and 2,914 milliliters per patient in 2001 (p < 0.05).
